Submit the following:
- Glen Oaks Community College Upward Bound Application.
- One letter of recommendation from your high school counselor.
- A letter stating your interest in the program and your college/career goals.
- A high school transcript and relevant standardized test scores.
- Family financial verification, a copy of 1040 form or proof of income benefits
(social security, public assistance, etc.).
- A completed Emergency Contact Information Sheet.
The Upward Bound Application is a PDF form and will require Adobe Reader. Adobe Reader can be downloaded for free below if you don't have it on your computer. You have the choice of printing the form out to fill in by hand or you can enter most needed information on your computer and then print it. Be sure to print 2 copies so that you can keep one.
